SLIDING FEE DISCOUNT PROGRAM

The Sliding Fee Discount Program is a federal program that allows us to discount our fees for medical services. Everyone is encouraged to apply, whether you have insurance or not.

The discount is based on household size and household income. To be eligible for the program, you must list each person in the household and provide proof of income.

You must. make an appointment with our Outreach & Enrollment Coordinator to complete the application. Call 907-224-2273, option 4 to schedule your appointment.

APPLICATION PROCESS

Make an appointment with our Outreach & Enrollment Coordinator by calling 907-224-2273, opt. 4.

Bring all required documentation to your appointment, including proof of income.

Complete and sign the Sliding Fee application form.

04

Your proof of income will be verified within 30 days.

If you meet income requirements, you will be placed in the program and receive a discount on SCHC medical services.  Discounts may be applied retroactively to your visits in the past 60 days.

If you are unemployed or do not have a verifiable source of income, you must complete a form explaining your current situation.
