APPOINTMENT POLICIES
CHOOSING A PROVIDER

Depending on your preference, your primary care provider may be a physician, physician assistant, or nurse practitioner.

You may request a male or female provider as your primary care provider (PCP). You may change your PCP by making a request at the front desk.

Your medical provider serves as a care team leader and coordinates all routine medical care, including exams, lab tests, medical imaging, diagnosis, and treatment.
INSURANCE

We accept all insurance plans, including private health insurance, Medicaid, Denali KidCare, Medicare, VA Choice, and TriCare.

We accept uninsured patients too, and can help you apply for insurance.
MAKING APPOINTMENTS

Appointments may be made up to 12 months in advance.

Same-day appointments are usually available.

If you need a same-day appointment, your primary care provider may not be available.

Patients under 18 years old must be accompanied by a parent or legal guardian.
PAYMENT

Payment (or co-pay) is due at the time of services.

If you cannot pay in full at the time of your appointment, ask about payment plans.

We also offer a sliding fee discount program for eligible patients.

List of medications you are taking

Proof of income for the Sliding Fee Discount
- Previous medical records and immunization records

Any additional paperwork or forms (disability, CDL, etc.)
CANCELLATIONS & NO SHOWS

If you need to cancel or reschedule your appointment, please call (907) 224-2273 24 hours before your appointment.

If you are late to your appointment by 20 minutes or more you may be asked to reschedule and you will be marked as a “No-Show.”

We will do our best to see you, but the scheduled appointments will take priority. You may have a long wait time.
