PAYMENT OPTIONS & PRICES
SCHC is not a free clinic. Payment (or co-pay) is due at the time of service.
If you cannot pay in full at the time of your appointment, ask about a payment plan.
Collection proceedings may be started if payment or payment arrangements are not made within 90 days of statements being mailed.

PROMPT PAY DISCOUNT
We offer a 20% Prompt Pay Discount to ALL patients who pay their self-pay balance within 30 days of the first statement date.

IN-NETWORK INSURANCES
We are in-network for the following insurances:

First Choice Health Network
Medicaid
Medicare (Part B)

VA Choice
We are certified out-of-network with TriCare, but working on becoming in-network.
UNINSURED PATIENT DISCOUNT
If you do not have insurance, you qualify for a 20% discount.
If you would like to apply for private insurance or Medicaid coverage, we can help you apply for insurance.
FEES FOR LAB & IMAGING SERVICES
Most lab & imaging services are provided by Providence Seward Medical Center, a separate company in our building.
You may receive a separate bill from Providence for these services.
It is not possible for SCHC staff to know how much those charges will be. Also, our staff is not able to accept payment for services provided by Providence.
SLIDING FEE DISCOUNT PROGRAM
Your visit may cost as little as $20 if you qualify and complete the entire application process.
