NEW PATIENTS
If you have not been seen by a provider at SCHC in the last three years (36 months), you are considered a new patient.

Call us to make an appointment at 907-224-2273, opt. 2.

Arrive 30 minutes early for your first appointment. This will ensure you have time to complete the new patient paperwork.

Gather the following items for your first appointment:

Insurance cards (if any)
-
Current medication list
-
Payment for your visit or co-pay
-
Your current address and phone number

SCHC accepts all insurance plans, including private health insurance, Medicaid, Denali KidCare, Medicare, TriCare, and VA Choice. We also offer a Sliding Fee Discount Program.

If your provider orders lab tests or imaging during your visit, you may receive separate bills from Providence Seward Medical Center. Please read more about our billing practices.
